Who are we
“Choices to Age at Home” including this website is owned and operated by Consultative Care Options Pty Ltd (ABN 75616463131). It is a privately owned, independent company with no affiliation to other organisations. “We” or “us” refers to the “Choices to Age at Home” organisation.

In these Terms and Conditions “you” are the user of this site and the client engaging us to provide services.
Acceptance of terms and conditions
“Choices to Age at Home” offers this website, including all information, tools and services available from this site to you conditioned upon your acceptance of all terms, conditions, and policies stated here. In engaging with us and our website and contracting our services it is taken that these terms and conditions are accepted in their entirety.
Changes
From time to time these Terms and Conditions may change. You should know that by continuing to use our site and services you’re agreeing to updated Terms and Conditions.
Engagement of Services
By requesting services from Choices to Age at Home (“we”) you engage us to provide aged care placement, aged care advice and advocacy services as outlined under Services Section on the website. Our role is to assist the Client in navigating the aged care system (residential and/or home care) which may include identifying suitable providers and facilitating the aged care process. We act as an independent consultant and do not provide legal, financial, or medical advice. This engagement commences on the date the Client accepts these Terms and Conditions and shall continue until the contracted services are completed or services are terminated. Whilst we provide expert guidance, we cannot guarantee a specific outcome or timeframe due to the nature of aged care which is dependent on external factors and external providers. For Transitioning to Residential Aged Care services, engagement ceases when a suitable bed offer is provided to the client for acceptance and the older persons moves into the residential home. If the client declines the bed offered and wants further beds to be sourced then a further engagement fee is required to be paid. This additional fee is required to be paid before further work commences. The Client acknowledges that we provide services to multiple clients and is not bound to provide services exclusively to the Client.
Client Obligations and Responsibilities
The Client agrees to provide us with all information, documents, and records necessary to perform Services in a timely manner. This includes but not limited to: My Aged Care referral codes and support plans; accurate financial disclosures (including Services Australia income and assets assessment) and medical histories and current care requirements. The Client is to ensure all information provided to us is true, accurate, and complete. The Client acknowledges that our advice and recommendations are based entirely on the information provided, and we are not liable for any outcomes resulting from inaccurate, withheld or misleading information. The Client acknowledges that we provide recommendations and options only. The final decision to enter into a care agreement or select a provider rests solely with the Client (or their legally appointed representative). The Client remains responsible for all fees or charges levied by aged care providers, medical professionals, or government agencies. Our fee is strictly for advisory and placement services and does not cover care costs. To ensure a streamlined process, the Client agrees to keep the Broker informed of any direct communications they have with aged care facilities or government departments regarding the placement.
Professional Conduct and Mutual Respect
We are committed to providing a supportive, professional, and respectful environment for all Clients. In return, the Client agrees to engage with the us in a professional and respectful manner. We reserve the right to suspend or terminate this Agreement immediately if the Client, or any person associated with the Client, engages in behaviour that is deemed:
- Abusive, harassing, or threatening (verbal or written);
- Discriminatory or derogatory;
- Unreasonably demanding or obstructive to our professional duties.
Should an incident of unacceptable behaviour occur, we will provide a written warning. If the behaviour continues, or in cases of severe misconduct, we may cease all services immediately. In the event of termination under this clause, all fees for work performed up to the date of termination will remain due and payable.
Protections against misuse or stealing content
You can use our site as long as you don’t hack us, steal or use our content without seeking permission and gaining approval or share your password with others or on-send content to others for whom it is not intended. You must not reproduce, duplicate, copy, publish, modify, adapt, transmit, distribute, sell, resell or otherwise exploit or use any portion of our website or materials without our express prior written permission.
Intellectual property protections such as copyright
You must not reproduce, duplicate, copy, publish, modify, adapt, transmit, distribute, sell, resell or otherwise exploit or use any portion of our website, materials or resources provided without our express prior written permission.
Safety and security on-line information
Our website uses a payment platform that securely allows you to select and pay for services. We do not store or keep any credit card or financial information. We collect only information that is required to provide the service you have engaged us for. This is collected through online forms and during consultations only.
Privacy
We collect and store information in accordance with the Australian Privacy Principles. Information shared with us is kept and stored confidentially and only the information required to provide the service you have paid for, is requested. It is expected if information is shared about another person or on behalf of another person that you have consent from that person to do so. Information provided
All information provided is general in nature. Information provided by us to you verbally and in writing is based on the information supplied by you. Our advice is general in nature and tailored for the individual older person who it relates to. We commit to ensuring information is as up to date as we are aware. Information provided in written format, video format and/or on this website is current at the time it was produced/provided. We recognise that the nature of aged care is that information changes regularly and is in the hands of Government Departments. We recognise that individual older persons circumstances may change rapidly and/or over time and information provided is current at the date and time provided but may not be suitable in the future due to changing health and other needs. We are not liable for any actions taken by clients based on information provided. We do not provide clinical, medical, financial, or legal advice. You are required to seek this information separately to our service.
Acting on behalf of someone else
If you are seeking advice on behalf of someone else it is assumed that you have full consent to do so including sharing their relevant health information. You are to be honest in your disclosure of your relationship with the other person. If the dealing is dishonest or elder abuse is suspected you may be reported to authorities.
Pricing
This website identifies the price for each service and what this includes. You are asked to think carefully before completing the purchase as refunds are not provided. Prices for our services may change from time to time and are subject to change without notice.
Rescheduling Appointments
There is the option to reschedule the consultation either for the same person or a different person. You can do this through our website.
Affiliations
As part of the service we offer, we may provide information on service and other providers as requested. We are not affiliated with any 3rd parties we are not legally bound by any agreement you may have with 3rd parties or the information provided by us on 3rd parties.
Complaints and Feedback
We work with our clients using an open disclosure framework. If you are unhappy with our service, please contact us using the contact form located on this website or phoning 1300 001 603 (Monday to Friday 9am to 5pm AEST) or so we can work with you to resolve.
Governing Law
These Terms of Service and any separate agreements whereby we provide you Services shall be governed by and construed in accordance with the laws of Australia.
